The TB test is mandatory for any UK visa application of 6 months or longer. South Africa is on the UK Home Office's listed countries, so there is no way around it.
What it is: The UK TB test for visa purposes is a chest X-ray, not a blood test or Mantoux skin test. Children under 11 have a clinical examination instead of an X-ray. The certificate is valid for 6 months from the date of the test.
Who needs it
- Anyone applying for a UK visa allowing them to stay for more than 6 months
- You must have lived in South Africa for 6 months or more immediately before applying
- Applies to all visa types: Skilled Worker, Ancestry, Spouse, Student and others
- Diplomatic passport holders on official postings are exempt

Cost
- Adults (11 years and over) — approximately R1,800-R2,200 depending on clinic
- Children under 11 — approximately R800-R1,200
- Fees are not refundable if you test positive or your visa is refused
The process
- Book at an approved clinic — Book online at register-uktb.iom.int (IOM) or contact your chosen clinic directly.
- Attend your appointment — Bring your passport. You will have a chest X-ray.
- Receive your certificate — If clear, you receive an IOM medical certificate confirming TB-free. Allow a few days.
- Submit with your visa application — Original certificate required. Valid for 6 months from test date.
Timing tip: Get tested 1-2 months before you plan to submit your visa application. This gives you time to gather other documents while ensuring the certificate remains valid. If your certificate expires before your visa is granted, you must be tested again.
